What is Endovenous Laser Ablation?

Endovenous laser ablation (EVLT) is a minimally invasive treatment that uses laser energy to close incompetent (leaking) veins responsible for varicose veins. A fine laser fibre is inserted into the diseased vein through a tiny puncture in the skin and, under ultrasound guidance, laser energy is delivered to heat and seal the vein from the inside. The treated vein gradually disappears as the body absorbs it over the following weeks.

How Does it Differ from RFA?

EVLT and radiofrequency ablation (RFA) work on the same principle of using heat to close the vein, but use different energy sources. Laser energy tends to produce slightly more post-procedure bruising than RFA, though both are highly effective and suitable for most patients. The choice between the two is often based on the individual anatomy of the vein and your specialist’s preference.

How is the Procedure Performed?

EVLT is performed under local tumescent anaesthesia in a clinic or day surgery setting. Ultrasound is used throughout to guide the laser fibre into position. Once correctly placed, the fibre is slowly withdrawn while delivering laser energy, closing the vein segment by segment. The procedure takes approximately 45 to 90 minutes depending on the length of vein being treated.

Recovery and What to Expect

Most patients walk out of the clinic shortly after the procedure and can return to normal daily activities the following day. A compression stocking should be worn for one to two weeks. Some bruising and mild tenderness along the treated vein is normal and settles within a week or two. Follow-up ultrasound is arranged to confirm the vein has closed successfully. Additional treatment of any remaining surface veins with sclerotherapy or phlebectomy may be recommended at a later appointment.
