Advanced Endovascular Management of Peripheral Vascular Disease

Advanced endovascular management refers to the use of minimally invasive, catheter-based techniques to treat complex peripheral arterial disease. This includes the use of drug-eluting balloons, covered stents, re-entry devices for chronic total occlusions, and atherectomy to restore blood flow to ischaemic limbs.

Complexity is determined by factors such as the length and calcification of the occlusion, the number of vessels involved, the quality of runoff vessels below the blockage, and the presence of critical limb ischaemia. Complex cases may require multiple endovascular techniques or a combination of endovascular and surgical approaches.

Advanced procedures may utilise drug-eluting balloons, drug-eluting stents, covered stent grafts, atherectomy devices (which remove plaque from the vessel), intravascular ultrasound, and specialised re-entry systems to cross chronic total occlusions. The appropriate combination is determined by the anatomy and severity of disease.

The primary goals are to restore adequate blood flow to relieve symptoms, heal wounds, prevent amputation, and improve quality of life. In patients with critical limb ischaemia, achieving limb salvage is the primary aim. Symptom improvement and reduction in amputation risk are the key measures of success.

Complex cases are approached using the most appropriate combination of endovascular tools available, with open surgery reserved for situations where endovascular approaches are not feasible or have failed.
